Explanation of Forms for Recording Activities

Purpose
- To document the activities involved in carrying out a comprehensive strategy, including the participation of specific partners
These forms:
- Help a partnership know whether it is on track with its plan for carrying out a comprehensive strategy. Answers the question, "Are we doing what we meant to do?"
- Chart the implementation of particular activities which are interim outcomes on the way to achieving the partnership's goals of improved results for children and families. Answers the question, "What have we achieved so far?"
- Track the participation of specific partners which improves accountability for interim results and enables the partnership to give credit where credit is due. Answers the question, "What is everyone's part in the strategy; is this what was promised?"
- Document "first time" events happening in the community, the building blocks for larger systems changes. Answers the question, "So, what have we done for the community lately?"
Who completes the forms?
- Sample Form for Recording Ongoing Services: Service providers associated with ongoing group activities that are part of a comprehensive strategy. For example, a social worker might report who attends a parent education series or a parent support group. A teacher might report who attends an after-school homework club.
- Sample Encounter Log (and Case Management Weekly Encounter Record): Service providers (specifically, case managers) who have contact with individual clients. A school counselor might report on sessions spent with particular students, a school nurse might report who received health screenings or services, or a social worker might report on clients he/she saw for service coordination.
- Sample Event Log: Designated participants in the comprehensive strategy, such as the coordinator and staff of key partners in the strategy. For example, a school nurse could report on a health fair sponsored by a school-linked services initiative, whereas a parks and recreation staff person in the same initiative would report on sports events sponsored through the initiative.
Who reflects on results?
- The entire partnership and community members.
How often are forms submitted?
- Regularly (e.g., monthly) to a designated compiler of the ongoing log of activities.
